The ED Care Coordinator RN job delivers discharge planning and clinical services to the Emergency Department (ED) during normal business hours and the entire hospital after hours. This role requires interaction with a variety of patient populations including, but not limited to neonatal, pediatric, adolescent, adult and geriatric. This role also provides direct intervention with patients and families and jointly with the health care team provides assessment, problem solving, advocacy and coordinate clinical interventions.
