Overview

Nurse Care Manager in Neurology will perform complex discharge planning for patients of different ages and developmental levels with serious illnesses. Focuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.

Qualifications: Bachelors degree in Nursing is required or Masters degree in Nursing, Health Care Administration, or related field. Three years nursing experience with two years advanced level pediatric clinical experience; one year utilization management and/or discharge planning experience.

License, Registration or Certification Required: Current Registered Nurse licensure in the State of California. Current CPR training Skills: Thorough knowledge of managed care, utilization management, discharge planning, and continuous quality improvement concepts. Ability to function independently as a clinician, role model, and patient/family advocate in a specialty area on a 24 hour basis. Computer Literacy. Well developed written and verbal communication skills. Strong interpersonal and negotiation skills. Fluency in both English and Spanish desired.
