The Nurse Case Manager in Endocrinology is a registered nurse who will manage patients with type 1 and type 2 diabetes mellitus. Will be involved in patient and family education, triaging of calls and teaching classes on diabetes basics and safety. Demonstrates thorough knowledge of managed care, utilization management, discharge planning, and performance improvement concepts. The Case Manager is a self motivated health care professional capable of performing complex discharge planning for patients with serious illnesses of different ages and developmental levels focusing on those patients with complexities related to clinical, finance and/or resource utilization components. The Care Manager will advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. The Care Manager will participate/facilitate in the development and outcome analysis of clinical pathways as a member of the multidisciplinary PI team. Demonstrates commitment to a patient and family focused model of care.
Minimum Education/Training Required
Bachelors degree in Nursing required: Masters degree in Nursing, Health Care Administration, or related field preferred.
Minimum Experience Required: Three years, nursing experience with two years pediatric experience in related field, one year of utilization management and/or discharge planning experience required.
License, Registration or Certification Required
Current Registered Nurse licensure in the State of California
Current CPR training and PALS through American Heart Association
Bilingual English Spanish preferred but not required.
