The Nurse Case Manager of Heme-Onc will perform complex discharge planning for patients of different ages and developmental levels with serious illnesses. Track chemo admissions, participate in daily rounds and communicate to key players, frequent monitoring of labs and communicate to providers, f/u radiology studies, initiate and follow up on prior authorizations, patient and family education on disease process, chemo plans and side effect profiles, address patient family concernsFocuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team. Qualifications: Three years nursing experience with two years advanced level pediatric clinical experience; one year utilization management and/or discharge planning experience. BSN required. License, Registration or Certification Required: Current Registered Nurse licensure in the State of California. APHON Chemo certification Current CPR training Skills Required. Thorough knowledge of managed care, utilization management, discharge planning, and continuous quality improvement concepts. Ability to function independently as a clinician, role model, and patient/family advocate in a specialty area on a 24 hour basis. Computer Literacy. Well developed written and verbal communication skills. Strong interpersonal and negotiation skills. Fluency in both English and Spanish desired.
