Position Summary: This Cardiology Nurse Care Manager role will be focused on Fetal Cardiology care. Collaborating with referring physicians, Cardiologists, CT Surgeons, and Fetal Maternal Center in the care of Expectant Mothers and their support system. Provides education on congenital heart disease and post-delivery care. Tracks patient’s progress through the health care systems by planning and implementing continuity of comprehensive care within in-patient, out-patient or home settings. Facilitates delivery notifications to inpatient units. Implement and manage quality dashboards with targets; report/track clinical outcomes.

Minimum Qualifications: 2 years recent pediatric cardiac experience required. Knowledge of congenital heart disease.

Education/Licensure/Certifications: Bachelor's Degree in Nursing (BSN) required; CA RN license, American Heart Association CPR and PALS certification.
