Job Description

Posting Note: Full time NP position for help in primarily out patient management of single ventricle patients during the time period they have shunt dependent physiology, typically between stage 1 and stage 2 of planned surgical palliations, as well as keeping our site up to date on data entry and management for our participation in the National Pediatric Cardiology Quality Improvement Collaborative (NPC-QIC). Duties include: - Oversight of home monitoring data on all patients enrolled in our program discharged from the hospital. - Provide direct phone access for outpatients enrolled in the program from 8AM to 5PM Monday through Friday. - Contacting families for any breech in “Red Flag” criteria to troubleshoot clinical issues. - Prior to initial hospital discharge- Educating families on the interstage home monitor program, goals for initial hospital discharge, and expectations for follow up visits and testing during the interstage period. Helping primary team assess discharge readiness. - Provide continuity of care by following patients while hospitalized during re-admission in the interstage period along with the primary team - Prenatal consultations as requested by fetal team - Weekly or as needed clinic visits (in person or by zoom) in the outpatient clinic Monday through Friday with primary clinic time once weekly on Wednesday afternoon - Keep current patient list up to date - Running the weekly multidisciplinary meeting for current patient review (out patients and in patients)- reviewing current clinical status, recent testing, follow up plan, procedure or surgical timing plans and any other concerns for the team. - Consenting patients for participation in and coordinating our site participation in NPC-QIC with RED CAP data entry. Participate in learning labs (webinars), enter patient data in RED CAP, attend twice yearly national meetings, help with quality improvement initiatives, and contact with parent liaisons. - Periodic (1-2 times a year) educational lectures for fellows and 5th floor nursing staff regarding our program and patients, SV protocols and updates for recommendations of care management - Planning and running our annual “graduation” event (once yearly) - Engage in research projects for institutional data analysis and quality improvement as time and interest allows Other duties currently include participation in the Fontan clinic and in patient neurodevelopmental rounds This position works in collaboration with the attending physicians and works within their scope of practice to provide excellent clinical care for patients. Position Duties Conducts comprehensive health history, physical assessments and plans for infants, children and young adults with acute and chronic health conditions. Utilizes advanced assessment skills to manage a cohort of patients. Orders and prescribes pharmacologic and non-pharmacologic interventions, and performs other therapeutic or corrective measures to treat illness and improve health status. Formulates family centered plan of care. Performs daily documents as necessary. Collaborated and coordinated care with other team members. Collects data and analyzes when necessary. Performs any needed advanced procedures that are associated with their credentialing process DOP. Utilizes the most up to date and evidence based practice for plan of care and able to alter the plan according to family needs and effectiveness or plan. Serves as an educational resource to patient/family/ nursing staff/ provider team and other health care teams as needed. Participates in own professional development by maintaining required competences, certifying body renewals, seeking educational offerings when available, and participates in the professional development of others which may include Advanced Practice Provider students, Residents and Medical Students. Acts as a preceptor/mentor for those new in department. Demonstrates commitment to patient safety by bring up concerns in appropriate manner, reporting through appropriate channels, and ensuring change of practice when necessary. Demonstrates shared accountability of safe practice. Performs miscellaneous job related duties as requested.
