RN New Patient Coordinator - Hematology/Oncology

• Reviews new patient referral records for completeness
• Reviews insurance and verifies it is in network with facility
• Establishes priorities of care
• Initiates requests for assistance when situations warrant it
• Assists administrative scheduling staff in determining how soon appointment should b scheduled Assists in updating new patient referral spreadsheet
• Reports monthly new patient referral data to administration
• Abstracts medical/surgical hx and medication list into EPIC
• Identifies patients appropriate for OCM and notifies Incentive Coordinators, attending physician, and Nurse Managers
• Input patient data into OncoNav
• Provides coverage for new patient scheduling as needed, for example, sick days or vacations Assists with phone calls, as needed
• Assists with scanning of new patient referral paperwork into EPIC
• Practices in a manner consistent with regulatory standards and hospital policy
• Collaborates and communicates with members of the healthcare team
• Utilizes evidence based knowledge to guide practice
• Reports to work as scheduled and maintains an acceptable record of attendance
• Performs other duties, as needed

NYS licensed/registered RN. Prior experience in an outpatient setting (specialty area) strongly preferred. Current CPR certification through the American Heart Association or the American Red Cross is required.

Monday - Friday 8:00 am - 4:30 pm
