It’s Work That Matters The Nurse Care Manager of Heme-Onc will perform complex discharge planning for patients of different ages and developmental levels with serious illnesses. Focuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team. Qualifications: Three years nursing experience with two years advanced level pediatric clinical experience; one year utilization management and/or discharge planning experience BSN or MSN required License, Registration or Certification Required: Current Registered Nurse licensure in the State of California. Current CPR training Skills Required: Thorough knowledge of managed care, utilization management, discharge planning, and continuous quality improvement concepts. Ability to function independently as a clinician, role model, and patient/family advocate in a specialty area on a 24 hour basis. Computer Literacy. Well developed written and verbal communication skills. Strong interpersonal and negotiation skills. Fluency in both English and Spanish desired.
