Job Description

The ED Care Coordinator RN job delivers discharge planning and clinical services to the Emergency Department (ED) during normal business hours and the entire hospital after hours. This role requires interaction with a variety of patient populations including, but not limited to neonatal, pediatric, adolescent, adult and geriatric. This role also provides direct intervention with patients and families and jointly with the health care team provides assessment, problem solving, advocacy and coordinate clinical interventions. Additionally, the ED Care Coordinator RN is accountable for utilization review for patients being admitted through from the ED.

Licensure, Certification, or Registration Requirements for Hire: Licensed Registered Nurse in the State of Virginia or eligible Licensure, Certification, or Registration Requirements for continued employment: Current Virginia RN license Complete 15 Continuing Education Units per year Experience REQUIRED: Minimum of three (3) years of clinical experience in an acute care setting to include specialty patient population Experience PREFERRED: Previous experience in acute discharge planning, home health, case management, long term care, or related community agency Education/training REQUIRED: Baccalaureate Degree in Nursing from an accredited School of Nursing Education/training PREFERRED: Masters Degree in Nursing ;from an accredited School of Nursing Independent action(s) required: Works with the ED attendings and consulting physicians to facilitate effective and efficient transition through the ED whether being discharged from the ED or admitted for additional hospitalization. Works collaboratively with all members of the interdisciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum. Seeks the expertise of Social Workers to resolve psychosocial patient care issues and to develop complex patient transition/discharge plan as needed. Interacts with patients, family members, healthcare professionals and community and state agencies in this effort. The ED RN Care Coordinator serves as an advanced clinical resource to the team. Serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information. This role is responsible for maintaining hospital compliance with the Quality Improvement Organization (QIO) and CMS guidelines. Utilizes medical necessity criteria as a tool to assess appropriateness of level and setting of care, assists in the denial and appeals process, assesses quality and identifies and reports potential risk management issues.
