Job Description

The Patient Family Centered Care Coordinator provides leadership and support in the educational and programmatic activities that promote and implement Patient and Family Centered Care. This job formally coordinates ways in which patient and family members can contribute to the mission and values of the health system.

Licensure, Certification, or Registration Requirements for Hire:

Licensed clinician (LPN, RN, PT/OT, etc.) in Virginia

Licensure, Certification, or Registration Requirements for continued employment:

Licensed clinician in VA

Experience REQUIRED: Minimum of two (2) years of experience working with patients and families and translating their strengths, needs and priorities into policy, procedures and operations. Minimum of three (3) years of clinical work experience

Experience PREFERRED: Minimum of 2 years of experience facilitating a patient-family advisory committee. Minimum of five (5) years of experience working with patients and families and translating their strengths, needs and priorities into policy, procedures and operations.

Education/training REQUIRED: Graduate of an accredited clinical Program

Independent action(s) required: Sound understanding of concepts of Patient-Family Centered Care. Capable of working with a diverse patient population. Displays passion for serving patients & families; strongly embraces the concepts of PFCC. Analytical ability to serve in an advisory/consultative role in determining or developing strategies, policies, processes, protocols, and methods. Demonstrated aptitude for fostering innovative approaches in a complex system. Exceptional communication and interpersonal skills to work productively with all levels of hospital personnel, to communicate effectively with diverse populations, and for frequent contact with patients and families in a variety of situations. Proficient in the use of Microsoft applications (Word, PowerPoint, & Excel) and experience with database systems. Ability to work independently, exercise appropriate action and good business judgement. Ability to manage multiple priorities, organize tasks, and maintain control of workflow. Ability to lead and motivate individuals and groups toward the accomplishment of assigned tasks and organizational goals.
