Job Summary: The Heart Failure Data Coordinator works with the Heart Failure Program Coordinator to assist with program data and education of patients who have a diagnosis of heart failure. Responsibilities include: coordinating with the "Get with the Guidelines" data abstraction, data entry and interpretation of heart failure program data and evidence based practice patient education. Collaborates in the implementation, coordination and evaluation of data for patients with heart failure. Coordinates and follows through with heart failure program data points and time targets. Will track and maintain database of all heart failure patients in the inpatient setting. Compiles and communicates quality outcome monitoring and data review and interpretation including time targets and heart failure improvement efforts.
Minimum Qualifications:

NYS licensed/registered (or eligible) RN required. Bachelor's degree in Nursing and two (2) years clinical acute care RN experience (preferably in Medicine/Cardiology) or Associate's degree in Nursing and four (4) years clinical acute care RN experience (preferably in Medicine/Cardiology) required. Excellent analytical, written/oral communication and interpersonal skills required.

Preferred Qualifications:

Experience with the "Get with the Guidelines" heart failure program data and data entry, heart failure standards and chart abstraction and data interpretation strongly preferred.
